Professional Vital Statistics Notebook

As something of an "old school" paramedic, I find it difficult to refer to the online chart on ePCR or similar when calling in a report of a patient I'm treating to a physician. This notebook and format makes it very easy for me to read off what I need to say to the doctor in a timely fashion. It is also easier to complete the report later with all the information, saving time to treat the patient instead of having to record it on a small computer, or use a smaller notepad where I have to write very tiny to get all the pertinent information down. One of the few suggestions I would make would be to incorporate blood glucose as a separate box in the vital signs area. Other than that, it is nearly perfect.
